The right output states what the document states and leaves the scale standing.
The same discipline applies to professional licences and continuing education records, which boards read against their own requirements hour by hour. Where a document runs to several pages with an annex, keep the annex with it: a transcript without its grading key is an incomplete document however well the rest of it reads.
